GOALS AND OBJECTIVES

GOAL 1: To reduce the prevalence of cardiovascular risk factors in the population of the province.
Objectives: 1. To use a community development approach for program development and implementation in order to ensure community ownership of cardiovascular disease prevention/intervention programming.
2. To increase awareness and action related to heart health issues in the province through policy development, professional education and public education.

GOAL 2: To diffuse (adopt, implement and maintain) selected intervention strategies into the existing community health system, in order to maximize program sustainability and impact.
Objectives: 1. To facilitate diffusion of programs through involvement of local health unit personnel in program development, implementation and evaluation.
2. To foster information exchange regarding program development through project linkages, media, NHHP Newsletter and Workshops.
3. To monitor the stages of diffusion (adoption, implementation and maintenance) and the extent of diffusion of heart health projects throughout the province during the life of the NHHP and for a 5-year period following its completion.
